If accepted you will receive a link to your Patient Packet. This includes a list of panels that must be completed prior to treatment.
STEP 3
Packet Review
Once your forms, medical history, and lab work have been submitted for review, our medical team will provide final clearance.
STEP 4
Book Treatment
Once approved, you may book your treatment on your desired date, subject to availability.
